Name this document as: last name, first name, student number.

If unable to save as one single document (due to size etc) please name as: last name, first name, student number (1), last name, first name, student number (2) etc.

Include in the body of your email your name, student number, university name, degree / diploma program name and year. If you do not provide your documents in this format you will be asked to re-submit.

Criminal Record Check documents

A copy of your criminal record check (and overseas police check / statutory declaration for international students) must be certified by a Justice of the Peace, scanned and saved (PDF preferred). Do not use zip files or transfer by any external file hosting service (eg Dropbox, SkyDrive).

Email from your university email address.

Include in the subject of your email your name, student number, university name.

Include in the body of your email your name, student number, university name, degree / diploma program name and year. If you do not provide your documents in this format you will be asked to re-submit.

Your immunisation compliance must be completed within six months of commencement of the degree / diploma / program and well in advance of any clinical placement. For most disciplines you must complete this in semester one. For those disciplines that attend clinical placement in the first semester of study (such as medicine) you will need to plan ahead to get this done before clinical placement.

Some vaccinations will take some time to complete so it is essential you commence as soon as possible. If you are unable to complete your hepatitis B vaccination and testing or TB screening we can allow you to attend your first clinical placement providing you have completed all your other immunisation requirements and have had at least the first dose of hepatitis B vaccination (see FAQ).

Criminal Record Checks (CRC) can take some time to obtain so it is recommended you apply for this before commencing your degree / diploma. Special provision has been provided for Pharmacy and Bachelor of Social Work (not Masters of Social Work) students at UNE to provide their Criminal Record Check at the beginning of their second year of study (as they do not attend clinical placement in year one). This may also apply to other degrees / diplomas which don’t undertake clinical placement in first year (by negotiation with the NSW Health agency responsible to validate your information in ClinConnect)

If you have not completed assessment requirements and had this verified in the ClinConnect database you will not be able to attend clinical placement.
